The following survey will assess usage of the Burke building. It should take no more than 10 minutes to complete.  The results of this survey will be made available to the community, following the completion of data collection and analysis. Survey data will be used in aggregated form only and all obtained information will be confidential Please do NOT sign this form.

Your completion of this survey is completely voluntary. You are free to leave any survey items blank.
